Often, it starts with a small oversight: a misconfigured system, an exposed file, or a permission that should never have been granted. In the AI era, however, the consequences of such errors are amplified dramatically, and the resulting incidents are often framed as AI-related threats or security vulnerabilities. A recent leak involving Anthropic’s Claude Code illustrates this shift. Researchers from Zscaler’s ThreatLabz found that a packaging error exposed an internal source map file, revealing portions of code that were not intended to be public.
